Subject - Let’s talk about death and dying - Community Conversation 1 Oct 21, 13.00 – 15.00 hrs via Zoom (A zoom link will be sent out to those who register nearer the time) Register by email to [email protected] The Hampshire and Isle of Wight End of Life Care Board is a partnership of health and care providers who look after people who are at the end of their lives. They have identified 5 key principles to underpin how they work together to provide good care and support to patients who are in the final stages of their life, their families, and carers. One of those is to improve how and when they identify people who are coming close to the end of their lives, to ensure they can care and support them and their families in a way that they would wish. To do that they need to understand what really matters most to people at that time. So they invite patients, families, carers, and groups that support them to join a community conversation to hear about what is important to you. The community conversation offers some time and space for people to share what could make a real difference to the experience of care for them and others, now and in the future. It will be facilitated by people with experience in helping people share their views and promotes active inclusion of people who wish to have a say. The stories they hear will be used to develop ways they can make a real difference. The conversation will be held on Friday 1 October, from 13.00 – 15.00 hrs using Zoom. It is open to anyone who is a patient, family member, carer or from a group who supports people at the end of their life Register by email to [email protected] Numbers for this event will be limited to enable all to have a voice and those registering will be advised once reviewed to ensure they enable representation from all areas. This is their first event and if there is a lot of interest, they aim to hold more in the coming weeks. The End of Life Board members have been invited to observe the session to enable to listen, really listen to what matters most to you.
