University Hospital Southampton have developed an online form which patients (or their carers) can complete to let them know if they have any accessible information needs when coming in to the hospital (such as interpreters, letters in braille/large print etc).

This can be found on the Trust website here: https://www.uhs.nhs.uk/PatientsAndVisitors/Patients-with-additional-needs/Preparing-for-your-visit.aspx

Once the form is completed, an alert will be added to their electronic Trust records which will flag these needs to their staff. They can also email [email protected] for advice or information about this.
